Top Notch Dentistry of Dallas is a dental practice located in Richardson, Texas, owned and operated by Dr. Kirmal Masih-Joseph. The practice is currently undergoing an ownership transition from Dr. Golbandi.
We have successfully encrypted their practice management systems and exfiltrated 2 GB of highly sensitive data from their primary workstation, including:
SERVER ACCESS & INFRASTRUCTURE
Server BitLocker Recovery Key – found on an employee desktop (we can decrypt their entire server at will)
PRACTICE TRANSITION & LEGAL DOCUMENTS
Signed Practice Sale Contract between Dr. Golbandi and Dr. Kirmal Masih-Joseph
Voided check and W-9 forms (bank account and Tax ID)
PROVIDER CREDENTIALS & LICENSES
DEA certificate for controlled substances
Credentialing applications for eight insurance carriers: Delta Dental, Cigna, MetLife, UnitedHealthcare, MCNA, DentaQuest, Careington, and Guardian
INSURANCE CLAIMS DATA

This data is currently stored on our secure servers. Top Notch Dentistry has been notified and given an opportunity to resolve this matter privately.
If no agreement is reached by the deadline, we will:
Publish all exfiltrated data
Notify the DEA regarding controlled substance record handling
Report HIPAA violations to the Office for Civil Rights (OCR)
Contact all eight insurance carriers listed above
Notify the Texas Attorney General's office
The ownership transition makes this breach particularly damaging, as buyer and seller are both exposed.
